Non-Profit Directors & Officers Liability Insurance

Term of Insurance: February 21, 2024 to February 21, 2025

Coverage limited to a claim first made while the policy is in force for a wrongful act and which is reported to the insurer no later than sixty (60) days after the termination of the policy.

PLAYER MEDICAL INSURANCE

All registered MYSA players receive medical insurance so if they are injured during a practice or game they may submit an Insurance Claim.
Term of Insurance: September 1, 2025 to September 1, 2026.

Insurance Claim Submission

An insurance claim form must be completed and submitted to MYSA within 30 days of the injury. Parents and club officials must complete the form. Once the form is received, MYSA will verify and authorize state approval for Player’s Health.
